Abstract:

The text reviews the use of prophylactic fluconazole in neonates, considering adverse effects and long-term consequences in patients. The increase in infections due to Candida spp. has been increasing, especially in children with very low birth weight (VLBW) and extremely low birth weight (ELBW). Candida albicans is the most isolated microorganism. The most frequent infections are Candida albicans, according to species parapsilosis, tropicalis, glabrata, lusitaniae and krusei. Risk factors for candidemia and invasive fungal infection are important for the detection and prevention of these infections. Preventing candidiasis in newborns is possible by using antifungal medications as early as possible. Studies have shown a significant reduction in candidemia cases and infant mortality rates. Antifungal drugs like nystatin, miconazole, and fluconazole have been studied for their effectiveness in preventing invasive candidiasis in newborns. Fluconazole is a promising tool in the fight against invasive candidiasis due to its ability to interrupt biofilm formation and prevent Candida colonization. However, the effectiveness and impact of fluconazole on neonatal mortality and resistance to antifungal medications remain a topic of debate. A bibliographic review was conducted on neonatology profileaxis for candidiasis using PUBMED and COCHRANE, focusing on recent advancements in the field from 2014- 2024. Prevention and treatment of Candida infections in premature neonates: general strategies include rigorous control, adequate management of central venous catheters, rational use of antibiotics, evaluation and minimization of modifiable risk factors, prophylaxis and positive evidence.

Biography :

Boris Pawlun earned his Medicine degree in 2013 from the National University of La Plata, Argentina. He specialized in Pediatrics and Neonatology at Evita Pueblo Hospital in Berazategui, Argentina, from 2014 to 2019. His medical training at Evita Pueblo Hospital spanned five years, including three years in pediatrics and two years in neonatology. He holds a Master in NICU from the Sociedad de Neonatología de España (SENEO). In 2018, he completed fellowships in the Neonatology Department and Cardiovascular Recovery (PCICU) at Sant Joan de Déu Hospital in Barcelona, Spain. Since November 2023, he has been working as a NICU specialist at Hospital Vithas in the Canary Islands, Spain. Prior to this, he served as a senior registrar in the NICU and CPR Coordinator at the Maternity and Children Hospital in Arar, Northern Borders, Saudi Arabia, from October 2021 to November 2023. From February 2019 until March 2022, he worked in the Pediatric Cardiovascular Intensive Care Unit (PCICU) at the Maternal-Infant Specialized Hospital in Mar del Plata, Argentina. There, he was responsible for the neonatology service and cared for pediatric and neonatal patients. Pawlun's extensive training and diverse international experience have equipped him with a comprehensive skill set in Pediatrics and neonatology.
