Abstract:

Pagets disease is an idiopathic, non neoplastic affliction characterized by disorded bone formation and resorption. It affects about 3% of the population in the European countries.There are no epidemiological studies or literature showing incidence of Pagets disease in the Asian population. Pagets disease affects mainly the skull, femor, tibia and pelvic bones. Facial skeleton is involved in about 17% of the cases. Nature of the disease is unknown, although evidence suggest that it is a multicentric benign tumor of the osteoclasts. The diagnosis of the disease is mostly based on radiological examination, biochemical markers of bone turnover and histopathological examination. Markedly elevated serum alkaline phosphatase is a constant feature while calcium and phosphate levels are typically within normal limits. We report a classical case of Pagets disease in a 28 years old male patient showing craniofacial deformity resulting in leontieasis (lion like face). Nasopharynx was completely obstructed due to bony expansion hence he is a mouth breather. Patient�??s eye moments are slightly restricted, no abnormality in the vision. On craniofacial radiologic examination which exhibited cotton wool appearance of the skull and elevated serum alkaline phosphatase level. Blood calcium and phosphate levels were examined in the patient which were within normal physiological limits. Histopathological examination revealed cellular proliferating inter-trabecular fibroblastic marrow with clusters of osteoclasts separating spicules of bone, which appear lamellar in some of the bits with numerous cement lines forming a mosaic pattern. Trabecula of bone with basophillic reversal lines in connective tissue sroma are seen. Currently patient is undergoing treatment.

Neha Bhaveshbhai Kaswala has cleared her final year BDS ranking 5th in the university and is currently doing internship from K M Shah Dental College and Hospital, S V University, Gujarat, India. She has presented an e-poster on veracity of virtual dentistry in international digital dental conference 2016, hyderabad. She has also attended annual world dental congress, FDI 2014. She was awarded with first prize for best poster presentation on “lab diagnosis of malaria”. She was awarded with 2nd prize in table clinics at Gyandeep 2016, SV University and also attended lecture series by Indian Society of Periodontology.
