Open and minimally invasive management of severe acute pancreatitis and its local complications: A 30-years experience in two centers with one tactical approach

Abstract:

Introduction: Worldwide, the incidence of acute pancreatitis ranges between 5 and 80 per 100,000 population. In the last three decades open procedures and minimally invasive techniques have been used to manage acute pancreatic necrosis and its local complications. Aims: The aims of this study were to quality of treatment on the role different procedures in patients with severe acute pancreatitis (SAP). Materials and Methods: A retro- and prospective two-centered controlled study was conducted in 1234 of both sexes from 18 to 70 yrs old who were treated in 1990-2020. Results: In the 1st group (410) as the standard surgical treatment we used open surgery with necrosectomy including programmed re-laparotomy in 13.3% pts. After surgery 21.2% pts were died. In the 2nd group (423) we used percutaneous catheter drainage and in 44.4% of them additionally used open surgical procedure from 2 to 4 weeks after onset of the disease. After surgery 20.3% pts were died. In 3rd group (401) 15.5% patients were treated by percutaneous controlled ultrasound and 42.4% pts were control video-laparoscopy pancreatic necrosectomy. In 1.1% pts were endoscopic treated by through the wall of the stomach or duodenum, in 25.2% by VARDs. In 34.2% patients with infected AP we were performed local approaches with pancreatic necrosectomy and drainage. After surgery 11.5% pts were died. Conclusion: Our study showed that using the stepup approach tactic reduces open necrosectomy and the number of postoperative complications and mortality.
