Lichen planus in the treatment of bipolar disorder: A case report

Abstract:

Lichen planus (Wilson�??s lichen planus, LP) is a chronic autoimmune inflammatory disease of not entirely determined etiology. The paper presents a course of interdisciplinary treatment which was introduced in a 46-year-old female subject diagnosed with a cutaneous and mucosal form of lichen planus. The patient was suffering from bipolar disorder and lithium salts therapy was administered. Clinical examination revealed erosions of the buccal mucosa and the ulceration of irregular shape on the lateral surface of the tongue. The lesions had persisted for six months prior to admission. Microbiological testing showed abundant microbial growth of Candida albicans and histological examination revealed features characteristic of classic lichen planus. Topical antifungal, anti-inflammatory, analgesic and protective treatment was administered. After two to six weeks no significant improvement of the mucous membrane was observed. Therefore, following a psychiatric consultation, a decision was made to terminate the treatment with a lithium preparation and replace it with another medicine applied in the treatment of the disorder. Discontinuation of lithium salt treatment resulted in healing the erosions of the buccal mucosa after four consecutive weeks. The ulceration present on the lateral surface of the tongue and cutaneous lesions remained unaltered. That was the reason why, after having consulted a dermatologist, systemic steroid therapy was implemented. The treatment was completed and the patient�??s condition has been monitored at the Outpatient Clinic of Periodontal Diseases at the University Dental Clinic in Krakow. Treating lesions of the lichen planus type confirms the necessity of implementing interdisciplinary cooperation among physicians of different specialties.

Weronika Lipska is currently working as a Didactic Scientific Staff in the Department of Periodontology and Oral Medicine Dental Institute, Jagiellonian University Medical College in Cracow, Poland. In 2007, she obtained the title of a Specialist in the field of Periodontology and in 2015, the title of Doctor of Medicine. She is the author of 25 publications in Polish and foreign magazines. Most of her work is devote to patients with Periodontal Diseases and with pathologies of the Oral Mucosa. She is interested in studying the relation between systemic diseases and medicines applied in their treatment and the state of periodontium and the oral mucosa.
