Abstract:

The common condition in elderly patients is the occurrence of edentulism, which can be the result of many factors such as poor oral hygiene, dental caries, and periodontal problems. Edentulism has the negative impact on oral hygiene related to the quality of life. Immediate function implant has become an accepted treatment modality for fixed restorations as patients demand an expeditious treatment plan for replacement of missing teeth. Severe atrophy of alveolar ridge often develops following tooth loss, with increased severity over time with the edentulous jaw. In cases to provide fixed implant-supported prostheses, an extensive surgical bone augmentation procedure is often necessary to achieve sufficient bone support to place standard implants (10-12 mm length, ~3.5mm diameter) in the posterior severely atrophic jaw. Augmentation procedure carries a higher risk of donor site morbidity and complications (e.g. Infections, loss of graft material) as well as higher costs. All on 4 concepts provides an option to place the implants in the ridge with different bone without going for grafting procedures. The principle of the all on 4 concepts is to use four implants in the anterior part of the complete edentulous jaw to support a provisional, fixed, and immediately loaded prosthesis. The two most anterior implants are placed axially, whereas the two posterior implants are placed distally angled to minimize the cantilever length and to allow the application of prostheses with up to 12 teeth. Final prosthetic solutions can either be fixed or removable dental prostheses.

Supreet kaur is the student of Sri Guru Ram Das institute Of Dental Sciences And Research, Amritsar. She is a member of Indian Dental Association (IDA) and has been actively participating in events held by IDA. She is also member of young research committee of SGRDIDSR, Amritsar.
