Abstract:

NIFTP is a new entity introduced after the 2016 reclassification of the tumour previously known as non-invasive encapsulated follicular variant of papillary carcinoma. This tumour cannot be definitively diagnosed preoperatively but can be suspected based on a castellation of ultrasonography, cytological, and molecular findings. Based on the available data from retrospective studies it should be expected that, when diagnosed based on stringent histopathology criteria, NIFTP should have a very low (< 1%) probability of recurrence. However, the indolent nature of this disease should be further confirmed in additional, preferably prospective series of patients, which should also evaluate the reproducibility and robustness of diagnostic criteria for NIFTP. NIFTP is currently considered a surgical condition, with thyroid lobectomy expected to be adequate for many patients and no immediate completion of thyroidectomy or radioiodine therapy recommended. Future studies should determine whether if an accurate preoperative diagnosis of NIFTP is possible using cytology and other ancillary techniques, such as molecular testing, and in what situations more conservative management can be safely offered to these patients.
