Abstract:

Molar Incisor Hypomineraliztion is a common congenital developmental anomaly resulting in enamel defects in the all upper and lower first permanent molars and upper and upper and lower permanent anteriors. This clinical report describes oral rehabilitation of a healthy 10-year-old boy diagnosed with molar incisor hypomineraliztion and it is the differential diagnosis. The patient presented with chief complaint of pain on chewing and severe sensitivity. Clinical examination revealed impacted upper left permanent central incisor and multiple carious teeth. In addition, the patient suffered of generalized gingivitis with poor oral hygiene. The patient was managed with multi displinary approach. Management included functional and esthetic considerations which resulted in an improvement in the patient's quality of life. The aim of treatment was to both restore esthetics and improve masticatory function. Treatment included composite veneers for all upper permanent teeth, adhesive restorations for all lower anterior teeth, extractions of unrestorable teeth, stainless steel crowns for all permanent first molars and space maintenance. Then followed with surgical exposure by apically positioned flap for the impacted upper left incisor. Finally, orthodontic extrusion of the impacted central incisor was achieved by bracket and chain connected to quad helix appliance until the tooth in position. Finger spring appliance was placed later to correct the cross bite and align the upper left central incisor in ideal position. Alternative treatment and future treatment plan will be demonstrated. No deterioration in the restorations and no pathology associated with the rehabilitation were found at the 2 years�?? recall.
