Histopathological merits to resolve diagnostic dilemmas when the parodontologist treats gingival epulis

Abstract:

Materials & Methods: The study was carried out in Periodontology Service at Mustapha Algiers University Hospital Center, on four patients suffering primarily from localized gingival overgrowth. In the differential diagnosis, mainly the patients' complaints, clinical appearance, nature of the lesions and histopathology were considered. Although dental radiographs were taken, looking for any pathology in the bone structure. Results: The primary complaints of the patients were the presence of overgrowths on the gingiva with bleeding and the secondary consideration was esthetic. Lesions were either sessile or pedunculated with different colors ranging from pink to red. The clinical appearance and medical history were very helpful for us to arrive at a pre-diagnosis of epulis and to identify of etiologic or predisposing factors. Furthermore, histopathological examinations of biopsy specimens after surgical excision have been required to confirm the diagnosis of pyogenic granuloma and peripheral giant cell granuloma discarding peripheral ossifying fibroma and metastatic cancer. All of the cases revealed dense inflammatory infiltrate, mostly mononuclear cells, covered with a stratified squamous epithelium. One peripheral giant cell granuloma displayed an appearance of high vascularization, resembling capillary hemangiomas and the histopathological evaluation contributed to the differential diagnosis. Conventional periodontal treatment has been sufficient. No complications and no recurrence of the lesions have been seen after postoperative controls. Conclusion: The determination of the microscopic nature of gingival epulis still seems to be the most accurate and reliable tool of diagnosis. The success of the ablative surgery needs compliance and patient motivation.

Omar Rahim is a Periodontologist. He completed his Post-graduation in Periodontology at Medicine University of Algiers Benyoucef Benkhedda. He practices periodontology service at Algiers’s Mustapha University Hospital Center as major postgraduate. He has devoted a large part of his time to study periodontal management of any diseases and published two articles.
