Abstract:

A Statement on Ensuring the Quality and the Safety of Medicinal Products to Protect the Patient was jointly signed by FIP and the International Federation of Pharmaceutical Manufacturers Associations (IFPMA) in 2000, according to which pharmacists are considered as drug therapy managers, plays a pivotal role in counselling, drug information services, drug therapy monitoring the cognitive functions of patient centred pharmaceutical services. The current prospective observational study of six months duration was designed with objectives to evaluate the actual and possible drug interactions, to categorise the severity of drug interactions, and to minimise the drug interactions in the prescription of patients in study site with 100 prescriptions. The actual drug-drug interactions found in the prescription are 12 which are minor. The possible drug-drug interactions are 56 of which 9 are major, 19 are moderate, 28 are minor. And 32 prescriptions don?t contain any interactions. Out of 100 prescriptions the actual drug interactions are 12% and possible drug interactions are 56% which can be assessed by clinical pharmacists and 32% of total prescriptions does not contain any interactions. Out of 56% of total drug interactions 16. 07% falls on major, 33. 93% moderate, and 50% are of minor drug interactions. In conclusion, the clinical pharmacists play a major role in assessing drug interactions to provide patient care, which is a new paradigm shift in pharmacy practice and patient care.

He is skilled in various areas of Pharmacovigilance including detection, assessment and monitoring of Adverse Drug Reactions and his research interest includes Pharmacovigilance, TDM (Therapeutic Drug Monitoring) and MTM (Medication Therapy Management) services.
