Esthetic soft tissue regeneration for mucogingival defects related to abnormal tooth anatomy

Abstract:

Malformation of the teeth structure can cause esthetic problem and affect children�??s quality of life. The present case report demonstrates esthetic management of gingival fenestration defect associated with an unusual root anatomy of a mandibular incisor by multidisciplinary approach. A 10-year-old girl visited our clinic with a chief complaint of gingival defect on right mandibular later incisor which worsened esthetics. Clinical and radiographic examination revealed the presence of additional cusp located on the root region and it appeared that the cusp contained pulp tissue. After a full thickness flap was elevated under local anesthesia, the additional cusp was carefully removed and the denuded root surface was recontoured. The exposed pulp tissue was covered with Biodentine (Septodont, Saint Maur des Fossés, France). Since insufficient alveolar bone support was expected, Emdogain (Strauman, Basel, Switzerland) was administrated on the denuded root surface to avoid gingival recession. Mucograft seal (Geistlich, Wolhousen, Switzerland) was adapted to the defect to induce gingival healing. And elevated tissue was sutured. Histologic examination confirmed presence of pulp tissue in the cusp. After one week, the defect was completely closed with healthy gingival tissue. The patient had not experience hypersensitivity and the tooth maintained the vitality of pulp during the follow-up period.

Ok Hyung Nam is a Clinical Assistant Professor in Department of Pediatric Dentistry, Kyung Hee University Dental Hospital, South Korea. He completed his DDS from School of Dentistry, Seoul National University. He is a PhD student in Department of Pediatric Dentistry at Kyung Hee University.
