Development and validation of training modules for patient medication history interview

Abstract:

Clinical pharmacy is a health science discipline which blends patient caring orientation with specialized therapeutic knowledge, experience, and judgment. In India, prevalence of well trained pharmacists is low, coupled with great variation in their training and regulation, which can be surmounted with problem based learning (PBL). With this in mind, a study was carried out to prepare, validate, implement and assure the quality of the training module for medication history interview in terms of provider, student and the quality assurance team. The study was prospective observational and was carried out for a period of six months at MCOPS and Kasturba Hospital, Manipal. Training modules were developed and validated on their relevance, completeness and appropriateness, which were used to train and evaluate six PharmD post baccalaureate students on skills needed for medication history interview (MHI). Performance of participants was evaluated before and after each training module. It was observed that theoretical module improved the knowledge of MHI process by 11%. The practical module was statistically significant (p value 0. 015* CI= -3.71 to -0.62) in improving the skills required. Feedback from participants ranged from good to excellent for both modules. Clinician feedback, on usefulness of MHI document in clinical setting (from average scores of 20 patients) was rated good. External audit and peer review reported good documentation and conduct of the theory, practical and practice components of the module. Implementation of this activity in the study hospital will help the clinical pharmacist to develop a rapport with patients and as well as physicians.

Kshipra K is doing Doctor of Pharmacy in Manipal University. Apart from Academic curriculum we are trained to do ward duties in 2400 bedded hospital which is attached to our college. We are providing clinical activities like patient counseling, taking medication history interview, provide drug information, Adverse Drug Reaction monitoring and reporting etc.
