Celiac not to be missed in infertility

Abstract:

Background: Celiac disease (CD) is a common chronic immune-mediated, inflammatory disorder of the small intestine induced by a permanent intolerance to dietary wheat, barley, and rye. Case Series: Ten married females, aged between 25-30 yrs, presented to infertility clinic with 5-8 year duration of infertility and could not conceive even after 3 cycles of ovulation induction. Out of ten six were totally asymptomatic and four had history of pain abdomen and generalized weakness. They were investigated in detail, including IgA TTG antibodies, which were found positive in significant amount in all of them. The endoscopic duodenal biopsies confirmed celiac disease, with modified marsh classification ranging from class 1-3. Result: Patients were put on Gluten Free Diet (GFD) and all of them conceived within 3-6 months. The pain abdomen and easy fatigability also reversed in four symptomatic patients. Conclusion: Celiac is becoming an important and neglected cause of infertility. Hence it must be considered in unexplained infertility, when all other causes have been ruled out. As reproductive alterations are reversible, a timely diagnosis and the introduction of a gluten-free diet are of paramount importance.

Vani Malhotra, MD, MICOG is working as Professor in Department of Gynecology & Obstetrics, PGIMS, Rohtak. She did her MD in from 2000-2003 and her thesis was commended. She is working as faculty at PGIMS, Rohtak, since 2006. She has fifty publications till date, both international and national. She is regularly presenting free papers, posters and chairing sessions at various national and international conferences. She is member of undergraduate and postgraduate board of studies at PGIMS, Rohtak. She is working both as Guide and Co-guide in many research projects. She is supervising many National projects, initiated at Gynecology and Obstetrics department of PGIMS, Rohtak.
