Abstract:

Objectives: To study the association between the mastoid process length and the success of surgery for chronic otitis media. Also, the association with hearing improvement, eustachian tube density and different grafting materials are also studied. Methods: The medical records of 179 patients were reviewed. The clinical presentation and diagnosis, preoperative CT scan, surgical procedures, and follow up details were recorded. Data were analyzed with proper statistical tests and validated with regression analysis. Results: Longer mastoid process length was associated with successful surgery for chronic otitis media. This was especially true for patients who had tympanoplasty without intact canal wall mastoidectomy (P= 0.014). Also, an improved air bone-gab closure results were associated with the longer mastoid process. Finally, there was a negative correlation between the mastoid process length and eustachian tube density. Conclusions: Long mastoid process, detected on temporal bone CT, is associated with successful surgery for chronic otitis media and improved air bone-gab closure.
