Primary Osseous Tumours of Spine and Pelvis in Children

Description

Primary bone tumours are cancers that originate in the bone. They may be benign or cancerous. Histological characteristics can be used to classify bone cancers. There are two types of benign tumours: non-aggressive and aggressive. The majority of bone tumours are benign. Osteosarcoma and Ewing sarcoma are the most prevalent malignant bone tumours in children. The anatomical position and origin of tumours of the spinal canal and vertebral column are classified. Intramedullary, intradural extramedullary and extradural spinal tumours account for 5%–10% of all juvenile central nervous system tumours when taken together. Brain tumours in children are six times more prevalent than spinal tumours in children. Extradural primary or metastatic tumours account for almost half of all spinal malignancies, followed by intradural intramedullary tumours (40%) and intradural extramedullary tumours (5%). In children and young adults, primary osseous spinal column tumours are uncommon, accounting for about 1% of all spine and spinal cord malignancies. These lesions can be benign or cancerous, and they can be difficult to diagnose and treat. Through the use of illustrative case scenarios, we examine the current methodologies for the care of primary osseous paediatric tumours of the spinal column, including benign and malignant lesions, and highlight clinical diagnosis, imaging, and surgical decision making.

Primary osseous tumours

Osteosarcoma develops in osteoid tissue from bone-forming cells called osteoblasts (immature bone tissue). This tumour most commonly affects children, adolescents, and young adults in the arm near the shoulder and the leg around the knee, but it can affect any bone, especially in older adults. It frequently spreads fast to other regions of the body, including the lungs. Children and teenagers aged 10 to 19 have the highest risk of developing osteosarcoma. Osteosarcoma is more common in men than women. Osteosarcoma is more common in blacks and other ethnic groups than in whites in children, while it is more common in whites than in other ethnic groups in adults. People with Paget Disease (PD) (a benign bone disorder characterized by aberrant bone cell formation) or a history of radiation to their bones are more likely to develop osteosarcoma. Osteoid osteoma and osteoblastoma are both benign osseous tumours that make up about 3% of all primary bone tumours. Osteoblastomas are larger than 2 cm, have more blood vessels, and are more likely to affect the vertebral body. Osteoid osteomas are 70 lesions with a diameter of less than 1 cm. Lesions between 1 and 2 cm in diameter cannot be distinguished only on the basis of size. Osteoblastoma represents less than 1% of all benign vertebral column tumours, whereas osteoid osteoma represents 9% of all benign vertebral column tumours. The spine is home to about a quarter of all osteoid osteomas and 40% of all osteoblastomas. Both of these lesion types are more common in the lumbar spine than in the cervical spine and can occur in the thoracic spine on rare occasions. The majority of these lesion types are located in the posterior elements. Males are more likely than females to develop osteoid osteoma throughout their second decade of life. Osteoblastomas are benign but aggressive locally, whereas osteoid osteomas are benign but aggressive locally. In reality, osteoid osteomas have the ability to "burn out" over time. Although osteoblastomas have traditionally been benign lesions, cases of malignant progression to osteosarcoma have been reported. The pathological findings of osteoid osteoma and osteoblastoma are similar. Both show bone formation by osteoblasts that produce osteoids and woven bones. However, these lesions can also differ in overall appearance and behavior during surgery. Osteoblastoma is a fragile, hemorrhagic mass lesion with a well-defined border to the surrounding bone, but osteoid osteoma has a strong membrane appearance and is high on Computer Tomography (CT). It is dense and expandable, with no evidence of bone destruction. Osteoblastoma often has a "ground glass" appearance on CT and is usually present in the cancellous bone of the cervical and lumbar vertebrae or vertebral roots. Bone-like tumors appear coarse, hard, scleral, and may have granulomatous components.

Conclusion

Primary bone spinal tumors are rare in children and young adults, but high suspicion indicators should be maintained when assessing children with low back pain with or without neurological dysfunction. Primary bone lesions in the pediatric spine can be benign or malignant and often have diagnostic and therapeutic challenges. A complete understanding of the clinical and radiological findings in each of these pathological conditions will help facilitate diagnosis and appropriate treatment. Surgical decision-making can be guided by a complete understanding of pathology. When it done properly, surgery can often result in a complete solution to pain and neurological symptoms.
