Description

The pork tapeworm (Taenia solium) larval cysts, which are sealed sacs harbouring the embryonic stage of a parasite are the primary source of the parasitic infection known as neurocysticercosis. This disorder known as cysticercosis is brought on by the larval cysts infecting different regions of the body. It is endemic in the majority of low-income nations where pigs are raised, and still one of the major factors in 30% of occurrences of epilepsy in endemic areas. Through fecal-oral transmission, this disease is contracted by eating food contaminated with the faeces of a T. solium tapeworm carrier. When there is a lack of hygiene, tapeworm eggs are passed in the stool, contaminating food. Eggs become larval cysts (oncospheres) when they are consumed and then exposed to gastric acid in the human stomach. Any organ can be impacted by infection.

In addition eating certain fruits and vegetables, drinking water infected with T. solium eggs, and contracting the disease from another person can all turn humans into intermediate hosts. The identical scenario as in the case of pork will take place, and the striatal muscle, subcutaneous tissues, central nervous system, and eyes will be the most negatively impacted organs. The condition is known as neurocysticercosis when it affects the central nervous system. The larval cysts can persist in the brain for several days despite only initially causing a mild immune response. A total, partial, or temporary restriction of Cerebro Spinal Fluid (CSF) flow is the main cause of the clinical appearance of the ventricular form of neurocysticercosis. Acute or chronic life-threatening ventriculitis can also be brought on by degenerating cysticerci adhering to the ventricular ependymal lining. Most often, periventricular inflammation, hydrocephalus, and, in more severe cases, a locked-in ventricle with impending herniation if addressed swiftly, are the signs and symptoms of ventricular neurocysticercosis. Because of its potentially fatal consequences and quickly evolving clinical course, ventricular neurocysticercosis is therefore regarded as a serious condition that requires urgent, aggressive therapies.

Depending on how long the host's immune system can tolerate them, the cysticerci can survive in the Central Nervous System (CNS) in their vesicular form for years or even decades. In contrast to when parasites reside inside the parenchyma, this typically asymptomatic latency phase appears to last much longer when it occurs in the extra parenchymal compartment. The cysticerci, however, proceed through several stages of involution once the host's inflammatory response starts. The initial stage is called the colloidal stage, and it is during this time that the cyst's inflammatory response is obvious, and the fluid inside the cyst also becomes turbid (granular stage). The inflammatory response subsides and occasionally is replaced by gliotic alterations as the cysticerci calcify (calcified stage) or vanish over time. By remodeling calcified lesions, cysticerci's antigens may become externalised, resulting in perilesional edoema that could be accompanied by clinical symptoms

Despite significant advancements in the treatment of Neuro Cysticercosis (NC) must to be conducted to determine the most effective management for a number of disorders. However, given what is known at the moment, the following suggestions can be made: To prevent worsening clinical symptoms in patients with vesicular parenchymal NC, anthelminthics should be administered in conjunction with corticosteroids. For patients with degenerative parenchymal cysts, it is unclear whether anthelminthics or simple follow-up is the best course of treatment. Treatment for calcified cysts should only be symptomatic. For extraparenchymal NC, focus should be placed on treating vasculitis (with corticosteroids) and hydrocephalus (with surgical alternatives such as endoscopic third ventriculostomy or VP shunt). In situations with ventricular cysts, surgical removal of cysts (mostly using endoscopic procedures) is a very good choice with the benefit of not requiring additional therapy if all cysts are removed.
